Scope change is not failure and preventing it is not the goal. It is going to happen.
The question is never whether to allow it. It is who is paying, and whether that person agreed. A change request is not paperwork. It is the invoice.
And a clear process for any change works both ends of the table. On the buying side it protects margin by making the cost visible before you agree. On the delivery side it is the revenue mechanism, because an enhancement that goes through cleanly is profit and one that gets absorbed is a gift.

Run a change through a process fast enough that nobody routes around it, and know whether it just cost your organization money or made it some.
Three modules, one month
Who is paying for that. Three kinds of change, three responses. A decision made too soon coming back is the system working. Something genuinely new is where the margin lives. Somebody relitigating a decision made properly is the only one worth resisting, and it is the one PMs let through because it arrives with the most confidence.
A process nobody routes around. Measure how long a small change actually takes. Then find out how long people think it takes, which is longer, and is the number governing their behavior.
Which end of the table. Price a real change from the other chair and find the margin you handed over, or the pushback you would have made.

Three lessons learned, tied to real projects, in your own words. What your project absorbed last quarter and which kind each one was. How fast your change process really is against how fast people believe it is. And a change priced from both sides of the table.
